How is the ending score calculated?
EndingScore = Discoveries × (+1) + Simon kills × (−2) + Cannula Refills × (−4) + surgery mistakes × (−3). Dialogue choices, time played and ordinary healing items do not change the score. The score is never shown in game, so count as you go; to work out where a run stands, add up your Discoveries and subtract 2 per Simon kill, 4 per Cannula Refill and 3 per surgery mistake, then compare with the thresholds below.

What is the Strange Access Card button order?
The hotel door button puzzle in New Game Plus uses the Strange Postcard hints. Press 21, 17, 9, 21, 12, 24, 24, 5, 3, 9, 1, 14, then take the Strange Access Card. This is PowerPyx's corrected order, published in its all-endings guide and checked here on October 15, 2026.
How do you see every ending with the fewest runs?
PowerPyx's roadmap uses two full runs plus reloads. There is no chapter select, so keep manual saves — dial 100 on any phone, see the codes page — at the points below.
- Run 1: collect everything and kill every enemy you can as Simon for Abandoned.
- New Game Plus: kill as few enemies as possible and collect the Clues and Documents again for Justice, while setting up the coins and books.
- Reload saves for Forever, Dog and Sci-fi. KeenGamer suggests a save from before the CEG Tower gate for Forever.
Which ending should a first run aim for?
There is no wrong first ending — the three main ones need no special route beyond the score, and collectible progress carries over between playthroughs, so a natural first run usually lands Justice or Forever. The five ending trophies are all missable in the sense that each ending needs its own run or save reload; plan the rest around the 85 Discoveries, tracked on the same page.
